The Centers for Medicare & Medicaid Services’ (CMS) Innovation Center, on Sept. 15, announced that the Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model will be expanded to include additional qualifying conditions. The technology-supported care option with outcome-aligned payments was originally limited to four conditions: early cardio-kidney-metabolic conditions, cardio-kidney-metabolic conditions, musculoskeletal conditions, and behavioral health conditions [refer to Washington Highlights, Dec. 5, 2025]. Beginning spring of 2027, the model will grow to include new tracks for heart failure, chronic obstructive pulmonary disease, substance use disorders, tobacco cessation, as well as an expanded follow-up period for bone, joint, and mobility conditions under the existing musculoskeletal track.
