Using Five Wishes to Facilitate the Advance Care Planning Conversation with Patients

As we transitioned our curriculum to remote learning to accommodate ongoing student learning during the coronavirus pandemic, we created a session to educate students on using 5 Wishes to facilitate the advance care planning conversation. Fourth year students were given a brief introductory lecture on advance directives and 5 Wishes, discussing the importance of advance directives, the barriers to having advance care planning conversations with patients, and details on how to approach this conversation with patients using a PowerPoint and two brief videos of physicians conducting advance directive conversations. Students then discussed their experiences related to advance directives and role played a part of an advance care planning conversation. This resource includes a PowerPoint, handout of phrases to use in an advance care planning conversation, two videos role modeling the conversation, and a facilitator guide for the session.
