CMS Announces Comprehensive Primary Care Plus Model

April 15, 2016— The Centers for Medicare and Medicaid Services (CMS) April 11 announced Comprehensive Primary Care Plus (CPC+), an advanced primary care model that builds on the foundation of the CMS Comprehensive Primary Care (CPC) initiative.

CPC+ is a regionally-based, multi-payer care delivery and alternate payment model that includes two separate tracks with different care delivery requirements and payment options. The goal of CPC+ is to support primary care practices to deliver high-value, comprehensive care in support of promoting healthy communities and reducing overall health care costs.

CPC+ is a five year model, beginning January 2017. The application process will be staggered for payers and practices. Beginning on April 15, 2016, CMS will solicit payer proposals to partner with Medicare in CPC+, until June 1, 2016. Based on interest and coverage, CMS will then announce the CPC+ regions and solicit applications from eligible practices from July 15, 2016, until September 1, 2016.

CMS anticipates partnering with commercial and state payers in up to 20 regions across the country, and will also engage with health information technology vendors to support the advanced CPC+ practices. Practices will apply to the track for which they believe they are ready. The CPC+ website includes more details about the Track 1 and Track 2 options.
