Documentation Requirements for MCAT AccommodationsThe following information is for applicants, evaluators, college and university staff, faculty and others involved in documenting a request for test accommodations. Applicants requesting test accommodations should share the guidelines with their evaluator so that appropriate documentation can be assembled to support the request for test accommodations.
Note: Applicants with chronic medical conditions (eg., diabetes, migraines, Crohn's disease, asthma) or temporarily-disabling conditions (such as a broken leg or recovery from an accident) should follow the guidelines for Documenting Physical Disabilities. For the protection of your private information, the MCAT Office of Disability Services will not accept inquiries from or provide information to third parties regarding your accommodation request, unless you have specifically authorized us to do so by completing the Authorization to Release Health Information to Third Parties (PDF, 1 page) .
