Believe in Better Health Toolkit

In October 2000, Strong Health launched "Project Believe" - an initiative to help Rochester, New York become the healthiest community in America by the year 2020. Strong Health's primary care physicians recognized their essential roles in supporting this lofty goal, and proposed a project to enhance and standardize patient communications regarding illness prevention, health improvement,
and risk reduction. The "Believe in Better Health" Toolkit of office and web-based tools was the
result.

Goals

  • To integrate preventive healthcare and health promotion into the core process of the
    health delivery system.
  • To identify risk factors for CAD, stroke, cancer, COPD and diabetes among others early to
    help prevent and reduce disease.
  • To provide a single standardized source of accessible education and referral information to
    improve access to preventive health information, promote health behaviors and reduce risk.
  • To enhance practice efficiency regarding wellness counseling.

Results

  • As of May 2003, more than 140,000 individual tools have been distributed to the 28 practices
    of the Strong Health Primary Care Network.
  • Measuring the impact of the Toolkit on patient outcomes.
  • Enhancing the Toolkit with updates and additions.
  • Making the Toolkit available nationally. A business plan is in development for mass production,
    underwriting, and national distribution.
