Application Forms

Applicants must submit a separate application for each PEP. Applicants who are responding to one of the research projects must apply using the PHS 398 grant application. Applicants who are responding to one of the non-research projects or who have an investigator initiated submission must apply using the PHS 5161 grant application.

Use of the modular budget format is prohibited when submitting any research, non-research or investigator initiated applications to AAMC, ASPH or APTR. All applicants must submit the detailed budget pages. Applications must not exceed the page limitations set forth in the instructions for either the PHS 398 or the PHS 5161.

Deadline

You must submit an original plus five collated copies of your application and appendices no later than 5:00 p.m. EST on March 5, 2007. Applications must be submitted to the Association in which your letter of intent was submitted.

Application Review Criterion

Modifications have been made to the research plan portion of both the PHS 398 and the PHS 5161 grant applications. These modifications will determine the score that your grant application will receive during the review process.

CDC does not permit direct contact between the Centers, Institutes and Offices (CIOs) and principal investigators. Any questions should be directed to your respective member association.


Submission Timelines

An original and five copies of your full grant application must be submitted to AAMC by 5 p.m. EST on March 5, 2007. Please be sure to read all of the application instructions prior to preparing your application.
