2007 Formatting Guidelines
PHS 398 Grant Application
The following PHS
398 grant application pages must be submitted for consideration. The
CDC follows the NIH's guidelines for point size and font. Point size should
be no smaller than 10 pt with Helvetica font and no larger than 12 pt
with Arial font. Please be sure to download the full
application cover sheet and attach it to each copy of your application.
Face Page - Must be signed by both the Principal Investigator
and the institution/organization's administrative official. The ID number
that you received during the LOI cycle should be clearly indicated.
Form Page 2 - Description/Performance Sites/Key Personnel
Form Page 3 - The Table of Contents must be modified to reflect
the sections required in the modified application
review criterion.
Form Page 4 - Detailed Budget for Initial Budget Period (Direct
Costs Only)
Form Page 5 - Budget for Entire Proposed Project Period (Direct
Costs Only)
Biosketches - A biosketch should be included for each individual
listed as key personnel on Form Page 2. There is a 4 pages limit for each
biosketch submitted.
Resources - A list of resources used must be provided.
Modified Research Plan - Section should include the following
six categories:
- A - D Address items in the application
review criterion (limit 25 pages)
- E - Human Subjects
- F - Vertebrate Animals
- G - Literature Cited
- H - Consortium/Contractual Arrangements
- I - Consultants
Checklist - Please submit your checklist with the appropriate
indirect rate indicated.
Personal Data Page on Principal Investigator/Program Director
- Submit one copy only as part of the original grant application.
Appendices - Provide an original plus six collated copies.
College and University Rate Agreement - You must use your institution's
federally negotiated indirect rate agreement. Please provide six collated
copies with your application.
