Current Status as of July 7, 1998
On May 8, 1998 the Health Care Financing Administration (HCFA)
published its proposed rule setting forth the changes to the
Prospective Payment System (PPS) for hospital inpatient services
for fiscal year 1999. The AAMC submitted comments on the proposal,
which were due July 7, 1998. A final rule is expected on or
about August 1, 1998.
Summary
The proposal contains the update factors for the various
payment rates within PPS, as well as a number of other changes.
Two proposals are of particular importance to teaching hospitals:
-
a change in the definition of "all or substantially
all" resident training costs that would require hospitals
to pay for resident travel and lodging expenses and physician
supervisory and teaching costs, in addition to residents'
salaries and benefits, in order to receive DGME and IME
payments for residents training in nonhospital sites.
- a provision that would permit HCFA to make payments to
certain non-hospital providers for direct teaching costs
if they incur "all or substantially all" of these
costs. (The nonhospital providers are Federally Qualified
Health Centers, Rural Health Centers, and Medicare Choice
organizations.)
AAMC Actions
- The AAMC has prepared a detailed summary and analysis
(hosp0026.htm) of the May 8 proposed rule. The Association
also submitted comments (corres/1998/070798.htm) on the
proposal on July 7, 1998.
